What we collect: Name, date of birth, contact information, preferred communication method, procedure interests, consultation notes, and voluntarily submitted photographs. We do not collect government health card numbers through this portal.

Why we collect it: To facilitate consultation planning, scheduling, care coordination, and communication between you and Dr. Jaberi's practice.

How it is protected: All data is stored in encrypted databases with access restricted to Dr. Jaberi and designated members of the practice. Data is not shared with third parties except as required for payment processing (Stripe) and communication services, each governed by their own privacy agreements.

Photos: Photographs you upload voluntarily are used solely to facilitate pre-consultation discussions. They are not part of your official medical record and will be permanently deleted following your consultation. Official medical photographs will be taken in-office as part of your clinical record.

Your rights: You have the right to access, correct, or request withdrawal of your personal health information at any time by contacting hello@jaberiplasticsurgery.com or calling 613-591-1188.

Applicable law: This practice is governed by Ontario's Personal Health Information Protection Act, 2004 (PHIPA) and Canada's Personal Information Protection and Electronic Documents Act (PIPEDA). We are also subject to the Canada Anti-Spam Legislation (CASL) — marketing communications require your express consent.

No Patient-Physician Relationship: Use of this portal does not create a patient-physician relationship. A relationship is only established following completion of an in-person consultation and mutual agreement to proceed with a care plan.

No Medical Advice: Nothing communicated through this portal constitutes medical advice, clinical assessment, or a recommendation for treatment. All information provided is for consultation facilitation and scheduling purposes only.

In-Person Consultation is Mandatory: A physical examination conducted in person with Dr. Jaberi is absolutely required before any surgical procedure or treatment plan can be confirmed. Online consultations supplement, but do not replace, the in-person assessment.

No Guarantee of Outcomes: Surgical and aesthetic outcomes cannot be guaranteed. Results vary by individual. Communications through this portal do not constitute any representation or warranty regarding results.

Cancellation & No-Show Policy: A valid credit card is required to reserve your consultation appointment. A $100 administrative fee will be charged for appointments cancelled with less than 24 hours notice or for missed appointments without prior notice. This fee reflects the administrative and clinical resources allocated to your appointment.

Appropriate Use: This portal is for the exclusive use of the registered patient. Sharing access credentials is prohibited. Any misuse may result in account suspension.

Governing Law: These terms are governed by the laws of the Province of Ontario and the applicable federal laws of Canada.
